Chapter 8 - The Siege of Northwestern

At 7:45 PM, the trauma bay at Northwestern Memorial Hospital was placed under absolute military lockdown.

Four armored vehicles smashed through the emergency room ambulance gates, parking directly across the bay doors. Fifty heavily armed men in dark overcoats surrounded the perimeter, clearing the waiting room in thirty seconds flat.

Surgeons, nurses, and orderlies stood paralyzed as Marco DeLuca walked through the automatic sliding doors.

He was a terrifying sight.

He wore no coat. His white shirt and navy trousers were entirely drenched in dark, sticky blood. His face was smeared with crimson, his eyes wild, red-rimmed, and burning with a lethal, desperate energy.

Behind him, two paramedics wheeled a gurney at top speed, pumping air manually into Lena’s lungs while three IV lines swung wildly above her body.

"Which one of you is the chief of trauma?!" Marco bellowed, his deep voice echoing off the sterile tile walls like a cannon shot.

A tall, grey-haired doctor named Dr. Aris stepped forward, his hands shaking slightly as he looked at the armed men surrounding the ER. "I am Dr. Aris. Mr. DeLuca, you cannot bring weapons into—"

Marco grabbed Dr. Aris by the lapels of his white coat, dragging the older surgeon until their faces were inches apart.

"That girl in there took five high-caliber rounds to save my mother," Marco whispered, his voice trembling with a deadly, quiet intensity that made the doctor’s blood run cold. "She has no family here. She has no money. But as of this second, she is the most important person in this city."

He pushed Dr. Aris toward the trauma bay.

"If she dies on your table," Marco added, his pale eyes fixing on the entire surgical team, "I will burn this hospital to the ground with all of you inside it. Do you understand my instructions?"

Dr. Aris swallowed hard, looking at the bloody form of Lena Carter being rushed into Operating Room 4.

"We will do everything humanly possible, Mr. DeLuca," Dr. Aris said quietly. "Get him out of the hallway!"

The surgical doors swung shut, the red IN OPERATION light illuminating above the double glass panels.

Marco didn't leave.

He stood directly outside the operating room doors, refusing a chair, refusing a towel, refusing to wash her blood from his hands.

Isabella sat in a wheelchair three feet away, wrapped in a blanket, accompanied by Caruso. She held a rosary in her trembling old hands, her lips moving in silent, desperate prayer.

Outside the hospital, the entire Chicago Underworld came to a dead, terrifying standstill.

News of the ambush at 31st Street had reached every crew, every captain, and every rival family within thirty minutes. But the detail that sent shockwaves through the city was not the attack itself—it was what happened afterward.

The Sovereign of Chicago—the man who had never showed emotion, who had never bowed his head to any living soul—was standing outside an OR, covered in the blood of a twenty-seven-year-old nobody caregiver, weeping while he waited for her to breathe.
