Chapter 3 - The Sanctuary of Glass and Steel

The private ICU on the top floor of St. Anthony’s Medical Center was built not for ordinary patients, but for men who lived their lives under the constant threat of assassination.

Reinforced steel doors replaced wood. Bulletproof glass lined the exterior windows overlooking the Chicago skyline. Outside the private elevator, four heavy-set men in dark suits stood guard around the clock, their eyes darting toward anyone who stepped off the floor.

Inside the sterile room, the steady, rhythmic beep... beep... beep of the heart monitor was the only sound breaking the silence.

Adrian Castellano lay in the center of the room, connected to three IV pumps, a ventilator tube in his throat, and a arterial pressure line taped to his wrist. His face was pale, his strong jaw shadowed by gray stubble, but his vital signs had stabilized thanks to the emergency surgical team that had spent four hours repairing his torn artery.

In the corner of the room, lying on a thick velvet blanket that had been brought from the mansion, was Titan.

The mastiff refused to move more than five feet from the bed. He ignored the nurses, the doctors, and the white-coated specialists who entered to check the monitors. He only responded to one person.

Elena sat on a low stool beside the bed, checking the line drip rates on her tablet.

She had been here for sixteen hours straight. Her shift at the contract agency had ended hours ago, but Marcus had made a single phone call to her agency's CEO, buying out her contract for the next month at five times her normal rate. She was no longer an agency nurse; she was now the personal medical guardian of Adrian Castellano—and the handler of his 150-pound shadow.

The heavy steel door swung open with a quiet hiss.

Lorenzo stepped into the room, holding a leather folder. He had changed his clothes, wearing a sharp charcoal suit, but his eyes were bloodshot and his expression was dark. Behind him walked a tall, thin man in a immaculate navy blazer—Julian Castellano, Adrian’s younger brother and the Chief Financial Officer of the family empire.

Titan’s head snapped up instantly. A quiet, dangerous grumble rolled through his chest.

"Keep that beast quiet," Lorenzo snapped, stepping back toward the door. "Julian needs to see the medical charts."

"He doesn't like the scent of your suit, Lorenzo," Elena said calmly, not bothering to look up from her tablet. "It smells like gunpowder from the estate. If you want him quiet, step outside."

Julian pushed past Lorenzo, his eyes cold and calculating as he looked down at his brother’s unconscious form.

"Is he going to wake up?" Julian asked flatly.

"He's under heavy sedation," Elena replied, standing up to check Adrian’s pupillary reflex. "The surgery went well, but he lost a massive amount of blood. The next forty-eight hours will tell us if there's any neurological damage from the hypoxia."

Julian’s eyes drifted toward the leather folder in Lorenzo’s hands. "The syndicate cannot operate without a leader for forty-eight hours. The docks in Newark are already stalling. The West Side distributors are asking questions about the gate breach."

"The gate breach was an internal job," Lorenzo muttered, his hand twitching near his coat pocket. "Someone bypassed the main security hub from inside the house."

"Which is why we need to sign the power-of-attorney documents now," Julian said smoothly, opening the folder and pulling out a set of legal papers. He reached toward the bed, aiming to press Adrian’s thumb onto an ink pad.

Titan stood up instantly.

With a deep, gut-wrenching bark that rattled the glass windows, the mastiff stepped between Julian and the bed, his head lowered, his mouth open in a terrifying display of teeth.

Julian froze, his hand hovering six inches from Adrian’s arm, his face draining of color.

"Don't touch him," Elena said, her voice dropping into that same quiet, unyielding tone.

Julian glared at her. "Excuse me? I am his brother. I am the second-in-command of this family."

"And I am the nurse in charge of this ICU," Elena replied, stepping directly beside Titan, her hand resting on the dog's massive shoulder. "Your brother is in a medically induced coma. His intracranial pressure is fragile. If you agitate this dog, his bark elevates Mr. Castellano’s heart rate to dangerous levels. Put those papers away and leave the room."

Lorenzo stepped forward, his face turning red. "You've got a lot of nerve, nurse! You think because you calmed a dog down, you rule this house? We can have you replaced in five minutes!"

"Try it," a heavy voice rasped from the doorway.

Marcus stepped into the room. The old security chief looked exhausted, his arm wrapped in a tight sling from his fight in the hallway, but his eyes were hard as iron.

"What did you say, Marcus?" Julian asked, turning slowly.

"I said try it," Marcus repeated, walking up to Julian and standing chest-to-chest with him. "Adrian gave me strict instructions six years ago. If he is ever incapacitated, security of his person falls entirely to me. And right now, the nurse and the dog are the only two living things in this city that I trust anywhere near his bed."

Julian’s eyes narrowed into dangerous slits. "Careful, Marcus. Power shifts quickly when a man is on a ventilator."

"Then wait until he dies to take it, Julian," Marcus spat back. "Because as long as his heart is beating, nobody touches that bed without Nurse Vance’s permission. Now get out."

Julian stared at Marcus, then at Elena, and finally at Titan, who was still letting out a low, steady growl.

Without another word, Julian snapped the leather folder shut, turned on his heel, and walked out of the room, Lorenzo following close behind.

The heavy steel door slid shut behind them.

Elena let out a long, shuddering breath, her shoulders sagging as she dropped back onto her low stool. Titan lowered himself beside her, resting his head across her lap once again.

"You made two powerful enemies just now, Elena," Marcus said softly, walking over to the window and looking down at the street below.

"I don't care about their power," Elena murmured, stroking Titan’s ears. "I care about my patient. Why were they in such a hurry to get his thumbprint?"

Marcus looked back at her, his face dark. "Because whoever turned off those gates at 11:31 PM didn't want Adrian dead just for revenge. They wanted his seat at the table. And if Adrian wakes up and tells us who had the master clearance code... someone in this family is going to die."
