Chapter 9 - The New Hierarchy

The response was instant and brutal.
Four of Julian’s own guards—men who had stood behind him ten seconds ago—turned on him, grabbing his arms, ripping the silk canopy down, and forcing him to his knees on the marble floor.
Lorenzo tried to run toward the side gate, but Titan moved like a black lightning bolt across the stone.
The 150-pound mastiff hit Lorenzo from behind, slamming him chest-first into the fountain. Titan didn't bite; he simply placed one massive, heavy paw on Lorenzo’s back, pressing the man’s face into two inches of water, pinning him effortlessly while Marcus walked over and slapped heavy steel cuffs onto his wrists.
"Please, Adrian!" Julian screamed from his knees, tears streaming down his face as his expensive suit was torn by the enforcers. "It was Lorenzo's idea! He wanted the docks! He forced me!"
Adrian stood over his brother, looking down at him with cold, pitiless eyes.
"You traded blood for money, Julian," Adrian said softly. "That is the one sin this family never forgives."
Adrian turned his back on his brother, walking back toward Elena and Titan.
"Marcus," Adrian said quietly.
"Yes, boss?"
"Take Julian and Lorenzo to the lower docks. Let the federal prosecutor have them. They'll spend the rest of their natural lives in a state penitentiary."
"Understood, boss," Marcus nodded, signaller for the guards to drag the screaming traitors away through the iron gates.
The courtyard fell into a profound, reverent quiet.
The fifty syndicate bosses, union leaders, and captains slowly approached Adrian, bowing their heads in absolute submission to the restored patriarch.
"Mr. Castellano," an old captain named Don Carlo said, stepping forward. "The family is yours again. What are your orders?"
Adrian looked at the fifty men who ruled the subterranean world of three states. Then, he looked at Elena, who was gently patting Titan’s head as the dog let out a happy, rhythmic pant.
"My first order," Adrian said, his voice clear and absolute, "is that this family is building a new wing at St. Anthony’s Medical Center. A twenty-million-dollar trauma center, fully funded, providing free care to every working-class family in this city."
Don Carlo blinked, surprised, but quickly nodded. "It will be done, Mr. Castellano."
"And my second order," Adrian continued, turning to face Elena, "is that Nurse Elena Vance is no longer a contract employee."
Elena looked up, surprised. "Mr. Castellano?"
Adrian walked over to her, stopping three inches away, looking down at her with a warmth that no man in that courtyard had ever seen him display.
"You told me in the cabin that you don't leave your patients until the shift is over," Adrian said softly. "Well, Nurse Vance... I am offering you a permanent position."
"As what?" she asked, a small smile touching her lips.
"As Chief Medical Officer of the Castellano Foundation," Adrian declared. "A salary of one million dollars a year, complete authority over our community outreach, and full security clearance across every property I own."
Elena stared at him, her heart skipping a beat. "Mr. Castellano... I'm a triage nurse."
"You are the woman who tamed a monster, saved an empire, and stood in the dark with a gun when thirty men ran away," Adrian said, reaching out to take her hand, pressing his knuckles gently against hers. "You are the strongest person in this city."

He looked down at Titan, who let out a deep, satisfied grunt, leaning his massive 150-pound body directly against Elena’s legs.
"And besides," Adrian added with a faint, rare smile. "Titan refuses to eat his dinner unless you're the one serving it."
