Chapter 3 - THE DOCTOR'S TONGUE

The storm raged fiercely as night fell over Oyster Bay. Inside the private medical office of Dr. Leonard Crane in Upper Manhattan, the lights were dim.
Crane was packing a leather briefcase with medical ledgers and private files. His hands were shaking slightly. Ever since he had left the Vale estate that morning, a knot of dread had been twisting in his stomach. The appearance of Mara Ellison had been a ghost stepping out of a nightmare he thought he had buried six years ago.
Suddenly, the heavy mahogany doors of his office burst open.
Two heavy-set men in black topcoats stepped into the room, followed by Gabriel Russo.
"Dr. Crane," Gabriel said calmly, his voice cold. "Mr. Vale requires your presence at the estate for an emergency consultation."
"It's eight o'clock at night!" Crane stammered, clutching his briefcase to his chest. "I have private appointments. If Adrian is feeling unwell, he can come to St. Jude’s in the morning—"
Before Crane could finish, the two men grabbed him by the arms, lifted him off his feet, and marched him toward the private elevator in the rear.
An hour later, Crane was pushed into the soundproof basement wine cellar of the Vale estate. The room was cold, lined with rows of expensive vintages and lit by a single overhead bulb.
In the center of the room sat Adrian Vale.
Beside him stood Mara Ellison, holding a tray of sterile surgical instruments that Gabriel had fetched from a local medical supply depot.
Crane stumbled forward, his hair disheveled, his expensive tweed coat torn at the seam. "Adrian! What is the meaning of this? This is kidnapping! You cannot treat me like this!"
Adrian did not look at Crane's mouth. He did not read his frantic pleas. He simply picked up a smooth, heavy silver pen from the table and wrote a single line on a notepad, shoving it into Crane's face.
Take it out.
Crane swallowed hard, looking at the notepad, then at Mara. "She's insane, Adrian! I am telling you as your physician, whatever she told you is a fabrication! There is nothing to extract! Performing surgery in an unsterile environment could cause permanent brain infection!"
Mara stepped forward. She didn't sign. She picked up a dry-erase marker and wrote rapidly on a whiteboard mounted on the cellar wall:
THE CAPSULE IS IMPLANTED IN THE OUTER MUCOSA OF THE MIDDLE EAR. IT CAN BE REMOVED WITH A STANDARD MYRINGOTOMY UNDER LOCAL ANESTHETIC IN TWENTY MINUTES.
Crane backed away, bumping against a rack of vintage Cabernet. "You're out of your mind! You're a disgraced resident! You haven't held a scalpel in six years!"
Adrian stood up. The sheer physical presence of the crime boss filled the small cellar. He walked up to Crane, grabbed the doctor by his silk tie, and dragged him over to the chair. Adrian forced Crane down into it, then looked down into his eyes with a coldness that made the doctor shudder.
Adrian pointed to Mara. Then he pointed to his own left ear.
He didn't need to speak. The command was absolute: Either she operates, or you die in this cellar.
Crane looked at the cold, clinical eyes of his former resident. He saw no mercy in Mara Ellison's face—only the sharp, burning desire for vindication.
"Fine!" Crane screamed, his voice breaking. "Fine! But don't touch me! Let me do it! I have the precise tools in my bag!"
Mara stepped between Crane and his bag. She picked up the dry-erase marker again:
NO. YOU WILL NOT TOUCH HIM. YOU WILL GUIDE ME THROUGH THE DEEP ANATOMICAL PLACEMENT YOU CREATED. IF YOU LIE, I WILL SEVER YOUR ULNAR NERVE.
Crane’s face went gray. He collapsed back into the chair, realizing that the castle of cards he had built six years ago was crashing down around him.
"It wasn't my idea," Crane whimpered, his eyes darting frantically toward the door. "I was in debt! The gambling dens in Atlantic City... I owed three million to the Moretti family. Celeste came to me. She said... she said if I helped her ensure Adrian remained dependent on her, she would clear my debts the moment they were married."
Adrian stood motionless as Mara translated Crane’s spoken words into rapid, precise sign language.
What about the bomb? Adrian signed, his hands shaking with restrained fury. Who planted the bomb that killed Thomas?
Mara turned to Crane and demanded the answer aloud.
Crane wept, burying his face in his hands. "It wasn't supposed to kill Thomas! It was supposed to just deafen you! Celeste's father had been ruined by your syndicate's takeover of the docks. She wanted revenge, but more than that, she wanted your money. She hired a crew from Brooklyn to set a small charge under your table. But Thomas... Thomas picked up the briefcase containing the charge thinking it was his own... he moved it closer to himself..."
Adrian closed his eyes.
A silent wave of grief, colder and deeper than the Atlantic Ocean, washed over him. For three years, he had believed his brother's death was a tragic casualty of syndicate warfare. He had hunted rival families, executed innocent lieutenants, and burned down warehouses in search of Thomas's killers.
And all the while, the monster who had ordered it was sleeping in his house, wearing a canary diamond on her finger.
Adrian opened his eyes. The grief vanished, replaced by a terrible, quiet clarity.
He turned to Mara, sitting down in the chair under the bright overhead light. He tilted his head to the side, exposing his left ear.

Mara picked up the local anesthetic syringe and the micro-scalpel. Her hands did not shake.
