Chapter 4 - The House Call

The suburban estate of Dr. Marcus Vance sat nestled in an affluent, gated community five miles north of the city center. It was a pristine, quiet street lined with weeping willows, manicured lawns, and high stone walls—the kind of neighborhood where wealthy men went to pretend their money was completely clean.

It was 5:15 AM when a sleek, black sedan pulled quietly into the dark driveway of the Vance residence, its engine gliding to a silent stop beneath the shade of two large oak trees.

Rocco stepped out of the front passenger seat, wearing a dark raincoat over his suit. From the rear door stepped two large, broad-shouldered men clad in black leather jackets—Paulie and Frankie, the syndicate's most reliable enforcers. From the other side of the rear seat stepped Amara.

Rocco had insisted she come along. “You're my expert witness now, little girl,” he had told her back at the library. “You identified the poison. You're going to identify the man who brought it into my boss's house.”

Amara pulled her thin jacket tightly around her shoulders, her heart pounding in her throat as she followed the three mobsters up the stone walkway toward the grand front entrance. The house was completely dark, save for a single light flickering in an upstairs bedroom window.

Paulie pulled a thin, flexible strip of steel from his coat pocket, slid it into the latch of the heavy mahogany front door, and with a soft, metallic click, unlocked the entrance. They slipped inside like ghosts, closing the door quietly behind them.

The interior of the doctor's house smelled of expensive cedarwood, polished silver, and aged bourbon. Expensive oil paintings lined the hallway, depicting serene European landscapes—a stark, absurd contrast to the brutal reality of the business that had paid for them.

They walked silently up the carpeted staircase, their footsteps swallowed by the thick pile. Rocco pushed open the door to the master bedroom without knocking.

Dr. Marcus Vance was fast asleep in a large king-sized bed, snoring softly beside his sleeping wife. He was a silver-haired man in his late fifties, with a manicured mustache and the soft, fleshy hands of a physician who had long since stopped doing real emergency surgery.

Paulie moved with terrifying speed. He stepped up to the bed, clamped a heavy, gloved hand tightly over Vance's mouth, and jammed the cold barrel of a suppressed pistol against his temple.

Vance's eyes flew open in absolute, blinding panic. He thrashing wildly, but Paulie held him pinned to the mattress as easily as a child holding a doll. Beside him, his wife stirred, but Frankie stepped around the bed, pressing a wet, chloroform-soaked rag over her nose and mouth until her struggling ceased and she sank back into a deep, unconscious sleep.

"Not a sound, Doctor," Rocco whispered, leaning over the bed until his face was inches from Vance's ear. "You make one noise, and I blow your knee off right in front of your wife."

Paulie slowly lifted his hand from Vance's mouth. The doctor was hyperventilating, his pale face covered in a sheet of cold sweat.

"R-Rocco..." Vance stammered, his voice trembling so violently his teeth chattered. "What... what is the meaning of this? Is Salvatore... did he pass away?"

Rocco reached into his raincoat pocket, pulled out the clear glass jar containing the six black threads, and dropped it onto the silk comforter directly in front of the doctor's face.

Vance looked down at the jar. In an instant, every ounce of blood drained from his face, leaving him looking like a corpse.

"Recognize those, Marcus?" Rocco asked softly, his tone terrifyingly pleasant. "They look a lot like the thread you used to close my boss's shoulder three weeks ago. The thread that's been leaking industrial cyanide into his heart for twenty-one days."

"I... I don't know what you're talking about!" Vance choked out, tears of terror welling in his eyes. "That was sterile surgical silk! I brought it straight from my medical kit! The wound must have become contaminated later—"

Before Vance could finish his sentence, Rocco reached out, grabbed the doctor's right index finger, and twisted it backward with a sharp, sickening snap.

Vance let out a muffled, agonizing shriek, but Paulie instantly jammed a heavy leather glove back over his mouth, smothering the sound into a pathetic whimper.

"Don't lie to me, Marcus," Rocco said, his voice dropping into a flat, deadly whisper. "A maid in our house—a girl who cleans floors—found those threads. She identified the chemical compound. She saved Salvatore's life. He's awake right now, Marcus. His fever is gone. And he's waiting for me to bring him a name."

Vance's eyes went wide with sheer, unadulterated horror. He stared at Amara, who stood near the bedroom doorway, her face pale but her expression cold and unyielding.

"I didn't have a choice!" Vance sobbed through Paulie's fingers as the enforcer loosened his grip slightly. "They... they said they would kill my children! They said if I didn't put those specific sutures in during the emergency procedure, they would burn my house down with my family inside!"

"Who is 'they', Marcus?" Rocco asked, leaning in closer. "Give me the name."

Vance shook his head frantically, tears running down his cheeks into his gray mustache. "If I tell you, they'll kill me anyway! You don't understand how far this goes! It's not just a rival family! It's—"

May you like

Rocco reached out and grabbed Vance's middle finger, preparing to break it.

"MARIO!" Vance screamed, breaking down completely. "Mario Bianchi! Salvatore's uncle! It was Mario!"
